REFERRALS / AUTHORIZATIONS

Most insurance plans require a referral to our doctors from your primary care physician in order for the visit to be covered by insurance. Some plans allow you to self-refer. In general, PPO plans allow self-referral, and HMO plans require a referral/authorization from the patient’s primary care physician. Check with your insurance carrier to see whether a referral/authorization is required. Please call our office to find out whether we have received your referral/authorization and to schedule an appointment.
