Welcome to our practice. Please read this page before you arrive for your appointment, as it will save time and give your doctor the information he needs. Please be sure to download and fill out the new patient forms at right. Thank you – we look forward to seeing you!

New patient visits

New patient visits usually last 30 to 60 minutes. The doctor will evaluate the patient, review records, and ask you about the patient's history. At the end of the visit, he will order any tests that might be needed to establish a treatment plan.

Insurance, co-payments and deductibles

Please remember to bring your insurance card and co-payment or deductible. Payment is expected at the time of your visit. We accept cash, VISA, MasterCard, Discover and American Express. For a list of insurance plans we accept, please click here.

To help us serve you better, please fill out the New Patient Form before you arrive for your appointment. You can download the form using Adobe Acrobat Reader. You can fill out the forms with your keyboard and mouse and print them, or you can print them and fill them out by hand.
